Unlike the “All‑on‑4” system, the All‑on‑6 technique is performed without incising the gums. Screws are placed in the strongest zones of the jaw: two in the front, two in the canine region, and two in the posterior sections. Built-in movable abutments simplify prosthetic fixation, making them more comfortable for the patient. 

Although All‑on‑6 costs slightly more due to the higher number of implants, its reliability and longevity make it an excellent choice for restoring full dental function.

Stages of prosthetic placement with the All‑on‑4 and All‑on‑6 methods

  • There is complete edentulism with pronounced jawbone atrophy due to natural resorption.
  • Inflammatory gum diseases such as periodontitis are present.
  • Teeth are completely destroyed or highly mobile.
  • Sinus lift or bone grafting is impossible or declined.

The choice of method depends on multiple factors, primarily bone volume and density.

For All‑on‑4, a larger bone volume is needed: minimum bone width of 5 mm and bone height of at least 10 mm in the upper jaw and 8 mm in the lower jaw. Bone density must also meet minimum requirements. All‑on‑6 has less stringent requirements, as six fixation points distribute loads evenly and allow for shorter implants.
